ALL Gnomes are “elusive.” That is their nature & modus operendi. “We” get to see them as nearly ubiquitous features because the Gnomes have great difficulty hiding from windows through the Fourth Wall.
Kohr, I believe, is referring here to the general knowledge within the Billverse that Gnomes are known to be just about everywhere, but nobody usually sees them or knows what they are up to. In this case, an Observer Gnome was briefly visible to Kohr as this cloaked figure appeared, apparently by some magical means (I speculate on that a bit further down in the comments).
The cloaked individual may not/probably does not even know the Gnome is with her, very much like the surprise appearance of the Gnome when Bill touched the Dark Underlord while time-traveling with Gypsy earlier in the story.
Notice the clothing and beard style of this Gnome … I think this is one we have not been introduced to (yet). Remember, there is an entire Syndicate that is Watching just about everything that goes on in the Billverse. This is probably not common knowledge among the denizens of this world. Possibly (?) this cloaked traveler is being accompanied by an Observer as she pops into the story she is overhearing/eavesdropping from the future? Perhaps even from the very tavern that Kohr is currently telling his tale in ..?

Cause and effect is maintained from the time traveler’s point of view, and a seperate timeline is created for everyone else for each temporal jaunt. Multiple universe theory. Cool.

How do you imagine Frank would sound if he was say an animated character? Would he have a voice that matches his size (sorta high pitched/nasal thing) or would he have the unexpected deep gravely voice?
a while back i suggested that if LOB was animated that Mike Greenberg (Greeny) of ESPN be the voice of Frank…and Bill Golic be the voice of Bill……i still like that idea
So far I don’t see anything that conflicts with what we know. It’s been obvious Bill has been dealing with some sort of amnesia. And I always figured Bill and Frank had been together forever. How else would Frank still be putting up with Bill these days?

That very person may well be showing up in the tale (in the past) as it is being told by Kohr right now in the present. As Kohr is telling it, the person is traveling backward and participating in it.
She could well be observing Bill while Foztik is distracted, both in the present and in the past using her grandmother’s incantation. I think it’s well-established at this point that Gypsy is quite a powerful magic user, even though we have only seen a few examples. The ability to time-travel, even as “only” an observer, is NOT trivial magic.
